Portal Registration

Our practice has created a secure electronic patient record for you, allowing you to log into our site, create and update your private health record.

Our free patient portal is available to you 24 hours a day, seven days a week, from the privacy and
convenience of your own computer.  The portal works best on a PC interface, but with patience, it works on your phone too.

This system helps us work together to maintain safe, secure, accurate and up-to-date medical records for you.
From now on, your important health information will be available to you when and where you need it, from any
computer with a high-speed internet connection.

Registration, security and access are very similar to those used for on-line banking. To register, you will be
required to provide the following information:
• Access Code: __________________________ (Sent to your email. Otherwise, please Text 970.674.0147 to request this code)

• Your email address (your email address will be your user name for log-in purposes)
• A password (you will create a password during registration)


To get Started:

1) Check your email and look for the message from: no-reply @medicfusion.com


2) If you did not receive an email, visit our patient portal login page.

- Click: "Not registered? Click here for our secure registration."

- You will need to text our office and ask for your access code. Text (970) 674-0147


3) After registering, (using 1 or 2 above) you will receive an activation email. This is very important! This email helps us verify that the

email address you provide is working. When you receive the activation email, follow the instructions to activate
your account and access it securely.

4) Look for the Patient login icon. After you register and activate your account, you can log in and look around

your electronic medical record. An on-page tutorial, located at the top-right of the page, provides a video tour of
the system and its features.

5) Click "My Profile in the upper Left Corner and you can update more of your information such as card on file and insurance information.

6) For Tax Records: Click Here.  Click the video link "How to find information to send to my FSA."
