Great news! Established patients can now use the form below to schedule their chiropractic visits without downloading the SKED app. Don't worry, the SKED app is still our main scheduling feature. SKED has simply created a web-based option too. In order for this option to work, your name and email must match exactly what we have in our system. If this function does not work, please fill out your name and email address on this form and we will update our records accordingly.
